Hypotensive cerebral ischemia (also called watershed or border zone infarction) is a relatively rare cause of ischemic brain alternation. Ithe underlying mechanism of borderzone infarcts is the low flow situation in the most distal field of brain supplied by the cerebral circulation. Hemodynamic impairment will consequently cause ischemia in these areas. The hypotensive cerebral infarction has a highly specific appearance in CT and MR images, which has to be recognised.
