The work presents a case of 23 year old woman admitted to the surgery department due to abdomen pain in right hypogastrium as a suspect appendicitis. The ultrasonography of abdomen was done. In the right hypogastrium there was a tubular shape with small anechogenous formations and small lymphatic nodes in the surroundings. It was concluded as appendicitis acuta dif.dg other. Due to unclear sonographic finding a CT of abdomen and pelvis minor was made and findings were: agenesis of the uterus, higher placed ovaria, dystophy of the left kidney and spina bifida. Appendicitis was not confirmed by the CT. The following gynaecological examination excluded gynaecological origin of the patient´s difficulties. The aim of this work is to highlight that even in the common diagnosis of acute appendicitis one should think of this rare syndrome in young women and that the agenesis of uterus and atypically placed ovaria can imitate appendix thickening.
